Family Opportunity Coach – Healthy Families
Salary: $36,000 - $39,500
The Family Opportunity Coach, or FOC, partners with families to work together on goals that will improve the family's self-sufficiency. The FOC is proficient in community resource knowledge and works as a liaison between the family and the resource, identifying potential supports for the family and providing more in-depth case management per the family's needs. The FOC is knowledgeable in Excel, reporting, building community partnerships and is self-motivated to ensure the family's needs are being met. This position supports the agency’s mission, vision, goals, and objectives.
This role requires travel throughout the state of Indiana.
The Family Opportunity Coach will:
Take initiative to meet community partners and to develop competencies to improve job performance.
Be proficient in Community Resources and act as the liaison between family and resource needed.
Maintain awareness of documentation timelines assuring that it meets professional standards and is submitted in a timely manner.
Be mindful of Family Opportunity Fund Reporting dates
Take initiative to meet ongoing training requirements, community partners and to develop competencies to improve job performance. Participate in the organization/planning of special program events
Advocate for parents and children in the community and participate in community special events
Position Requirements:
High School Diploma or Equivalent
Three (3) years relevant experience (combination of education and experience may be accepted)
Bilingual in Spanish and English
